About the Role Were looking for a highly motivated and qualified Clinical Pharmacist (Independent Prescriber) to join a friendly and supportive GP practice within Oxford PCN.
Youll play a key role in improving patient care by providing expert pharmaceutical advice, resolving medication queries, and supporting the smooth day-to-day running of the practice. Working as part of a multidisciplinary team, youll contribute to better patient outcomes and improved access to healthcare.
Key Responsibilities
Resolve medication-related queries for patients and practice staff
Conduct polypharmacy and structured medication reviews (SMRs)
Perform face-to-face medication reviews and post-discharge reconciliations
Provide clinical leadership on medicines optimisation and quality improvement
Offer expert pharmaceutical advice to clinicians and patients
Support audits, service improvements, and QOF/CQC workstreams
Promote safe, effective, and efficient use of medicines
Educate patients on medication use and disease prevention

Requirements Essential:
GPhC-registered Clinical Pharmacist with Independent Prescribing qualification
Completion of CPPE Pathway or Exemption
612+ monthsexperience in a GP practice or primary care setting
Experience using EMIS or SystemOne
Living within a commutable distance to Oxford
Additional:
Understanding of QOF, CQC, and local incentive schemes
Enhanced DBS check required (in line with Rehabilitation of Offenders Act 1975)
