Overview
We are looking for a Senior Practitioner to work within Stamford Out of Hours delivering face to face consultations and telephone triage.
We are looking to recruit dynamic individuals with minor illness skills that will also compliment the existing team of GP's, ACPS and HCSW's.
Key Responsibilities
* Deliver face-to-face consultations and telephone triage service 19:00-22:00 Monday-Friday, 08:00-22:00 weekends and bank holidays.
* See, treat, manage, discharge and refer patients of all age ranges to LCHS services or partner services across Lincolnshire.
* Support urgent care services such as UTC's, home visiting and the Clinical Assessment Service (CAS) which run 24/7 services.
* Document consultations timely and thoroughly within patient notes.
* Influence, lead, support and motivate colleagues to maintain morale in challenging situations.
* Provide clinical lead during a shift by modelling professional attitudes and behaviours outlined in the LCHS behaviours framework.
Qualifications
* Recognised qualifications and experience in minor illness and/or experience working in GP practice.
* Hold a non-medical prescribing qualification or willingness to work towards.
* Excellent leadership and communication skills.
