The post holder will be employed as a member of the Community Children’s Nursing Team and will have continuous responsibility for the management of a caseload. He/she will have continuing responsibility for the assessment, development, implementation and evaluation of programmes of care.Please see employer website for full job details & person specification.
Responsibilities
* Support families as necessary in the community through home visiting, referral to other agencies.
* Promote the management of children in the home, school or community by supporting and training carers to meet the individual needs of children.
* Provide support to families/carers caring for a child with chronic illness and/or disability or long term illness.
* Carry out and complete the initial nursing assessment on all Children with complex nursing needs.
* Be responsible for establishing and maintaining an inter-professional/multi-disciplinary communication network which will facilitate service development, and facilitate close links between the child’s key worker and the Community Children’s Nursing.
Skills and Qualifications
* Registered Nurse on Part 1 of the NMC Register (Children’s).
* 3 years’ experience in children’s nursing.
* BSc (Hons) Community Nursing (Community Children’s Specialist practice option). Those committed to completing the Community Children’s Nursing Qualification within 2 years may also apply.
* Ability to work flexible hours including evening, night time and weekend work if required.
* Hold a current full UK driving licence and have access to a car or form of transport that will enable the post holder to carry out their duties.
* ‘Where disability prohibits driving, this criteria will be waived if the applicant is able to organise suitable alternative arrangements.’
* Experience of working with children with disability and/or chronic illness/palliative care.
* Additional experience working as a Band 6 in a children’s nursing setting.
* One year’s experience in Community Children’s Nursing.
