Job Overview
The Surrey Downs Health and Care Community Therapy Team works with GP Primary Care Networks and provides rapid access therapeutic/MDT intervention for those who would otherwise need emergency hospital admission, and for those medically stable to return home from an acute hospital but requiring enhanced short‑term support as one stage in their ongoing care pathway. The Community Therapy Service offers rehabilitation and falls prevention to patients in their own homes. Working closely with rapid response services under the Urgent Community Response initiative, the team ensures a joined‑up, co‑ordinated care provision to local citizens with health and rehabilitation needs, supporting people to live as independently as possible through multidisciplinary, holistic assessment and further rehabilitation to reduce admissions to the acute system.
Main Duties of the Job
* Perform advanced physiotherapeutic assessment of patients with complex presentations using advanced clinical reasoning skills to create targeted treatment plans using SMART goals specific to the patient’s condition.
* Formulate and deliver individual or group physiotherapy treatment programmes in line with best practice guidelines.
* Co‑ordinate and ensure effective delivery of therapy services by liaising and cooperating with colleagues across other PCNs and adult social care teams to provide equitable services.
* Evaluate patient progress, reassess and alter treatment programmes as required and advise and support more junior colleagues. Hand over treatment plans to the rehabilitation assistant to support patient‑centred goals.
* Refer to local community services appropriately and in a timely manner.
* Understand and manage clinical risk within own patient caseload and within the service, advising more junior staff and seeking guidance from Band 7s or the Clinical Lead as required.
* Keep accurate and complete records of work undertaken in line with CSP, Community Therapy Team, Surrey Downs Health and Care standards, maintaining confidentiality and information governance standards at all times.
* Teach, assess and contribute to the performance assessment of Band 5s and Rehabilitation Assistants.
Qualifications
* Degree or diploma in Physiotherapy. Registered with the Health Care Professions Council.
* Membership of the CSP.
Experience / Knowledge
* Relevant post‑qualification experience across a range of client groups, appreciating constraints to working in the community.
* Experience of MDT working, clinical teaching within the MDT, supervising and working with juniors, assistants and students.
* Demonstrated ongoing CPD and evidence‑based practice.
* Experienced manual handling skills and knowledge of relevant health, safety and risk issues.
* Post‑qualification experience working in the community, clinical audit, research and literature reviews.
* Proficiency with electronic notes recording systems (e.g., EMIS and Systm1), and basic Excel and Word.
Skills
* Physical ability to carry out therapy assessment and intervention.
* Valid driver’s licence with vehicle for business use.
