An exciting opportunity has arisen for an enthusiastic and motivated Community Nurse/Health Facilitator to join the Adult Community Learning Disabilities Team (CLDT)Trafford, based at Sale Waterside.
The CLDT is a well-established and friendly team comprising of nurses, speech and language therapists, occupational therapists, physiotherapists, psychologists and psychiatry.
Nursing is a valued and respected component of the team. The team also links closely with our colleagues in Greater Manchester, as part of a wider Greater Manchester Network.
The Community Nurse will work as part of the multi-disciplinary community support team, and will be expected to work autonomously, managing a defined caseload of adult service users with Learning Disabilities and complex health needs.
The post requires an enthusiasm and commitment to working with adults with learning disabilities within a person-centred approach.

Managing a caseload under supervision, the post holder will be expected to work autonomously and as part of the team to improve outcomes for adults with Learning Disabilities within their local community, whilst maintaining quality lives and ensuring people are free from harm.
Applicants should possess excellent interpersonal, communication and literacy skills and the ability to work with a range of professionals, agencies and carers in a wide range of settings. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.
Development opportunities will be provided in the form of training and you will receive regular clinical & management supervision and annual appraisal.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
