We have various exciting opportunities in the Stockton-on-Tees 0-19 Service, working within our Health Visiting Teams for Healthy Child Practitioners, Community Nursery Nurses, and Early Years Practitioners.

Main duties of the job
The Healthy Child Practitioner is a member of the 0-19 Integrated Public Health Service (IPHS), working within the policies and procedures related to the 0-19 service.
The post holder will work closely with the SCPHN within a skill-mixed team to support the delivery of the 0-19 Service Specification and Healthy Child Programme (HCP).
The service aims to provide universal access to children, young people, and families to the Healthy Child Programme and facilitate early identification of additional and/or complex needs.
The post holder will contribute to the Public Health Agenda, supporting health promotion and reducing health inequalities, in partnership with key agencies to meet local and national objectives.
