JOB-20240819-db742659 Adults Social Worker - Hospital Discharge Team Specialism: Social Worker - Adult's Location: Sheffield, UK Pay rate: £30.00 per hour Salary type: Hourly Contract: Ongoing Working pattern: Part Time / Full Time Role overview An exciting opportunity has arisen for an experienced Adults Social Worker to join the Hospital Discharge Team in Sheffield. This ongoing locum role offers both part-time and full-time options, providing flexibility while allowing you to make a meaningful impact supporting safe and timely hospital discharges. You will play a vital role in ensuring individuals receive appropriate care and support following discharge, working within a fast-paced hospital environment and collaborating closely with multidisciplinary professionals. Supportive Team Environment: Work within a collaborative and experienced multidisciplinary team. What you will do Conduct Care Act assessments for hospital inpatients and develop timely, effective discharge care packages. Complete Mental Capacity Assessments and contribute to Best Interest decisions where required. Manage safeguarding enquiries in line with statutory responsibilities. Demonstrate knowledge of Section 117 of the Mental Health Act and participate in associated meetings, including ward rounds and professional conferences. Participate in the team duty rota to ensure consistent service delivery. Maintain accurate and up-to-date case records using the LAS (Liquid Logic) system. Work collaboratively with health professionals and partner agencies to support safe discharge planning. About you Qualified Social Worker registered with Social Work England. Experience within hospital discharge and/or adult mental health settings. Sound knowledge of the Care Act 2014, Mental Capacity Act, and Section 117 processes. Confident managing complex cases in a fast-paced environment. A car driver is preferred due to potential travel requirements within Sheffield. Why Sheffield?
