Join to apply for the Practice Nurse role at NHS.

Job summary An exciting part-time Practice Nurse opportunity at Helmsley Medical Centre and Terrington Surgery, due to a planned retirement. Open to experienced practice nurses, new or recently qualified nurses, or those new to general practice considering this field. We can enrol a new Practice Nurse into the vocational training scheme with a year of supported teaching and learning alongside working in the practice. Applications from community nursing or secondary care are welcome. This role focuses on delivering high-quality nursing care to the practice population under supervision of Senior Nurses.

Working pattern
17 hours per week with rotas across Helmsley Medical Centre and Terrington Surgery to be agreed.


Main duties and responsibilities

* Meet patients’ nursing care needs, usually in the surgery with occasional home visits; assess, plan, provide and evaluate nursing care to meet needs in the practice population.
* Work with the multidisciplinary team to meet patient needs and contribute to key performance indicators for Helmsley and Terrington Practices.
* Provide general nursing care including wound management, chronic disease management, vaccinations and immunisations for babies, children and adults.
* Provide women’s health services including cervical screening and contraception.
* Conduct health assessments and screenings; support patients with long-term conditions via education and self-management advice.
* Participate in health promotion activities and initiatives; maintain accurate patient records.
* Co-ordinate and deliver travel medicine services as required; review patient medications for compliance.
* Assist with chronic disease clinics, refer patients to secondary/specialist care as required, and support safeguarding and clinical governance.
* Assist GPs with minor surgery when required and respond to clinical emergencies as part of the team.


Secondary key responsibilities

* Participate in local initiatives to enhance service delivery and patient care.
* Support shared learning within the practice and review clinical practices against national policies.
* Participate in the review of significant and near-miss events using structured approaches (e.g., RCA).


Person specification

Essential

* Understanding of evidence-based practice and ability to record accurate clinical notes.
* Ability to work within scope of practice and know when to refer to GPs; use of chaperone procedures.
* High standards of health promotion and public health awareness; strong communication and IT skills.
* Flexibility to work outside core hours; DBS check; Occupational Health clearance; evidence for nurse revalidation; CPD aligned with the role.
* Registered Nurse with NMC registration.

Desirable

* Additional qualifications (e.g., mentor/teaching qualification, postgraduate diploma/degree in Chronic Disease Management, family planning).
* Experience in wound care, ECGs, venepuncture, immunisations, women’s health, pathology tests, travel medicine, chronic disease management.

Experience

* Essential: experience of working autonomously; infection prevention and control.
* Desirable: experience as a practice or community nurse; managing elderly/housebound patients; quality initiatives; primary care environment.

Skills

* Excellence in communication (written and oral), time management, teamwork, and independent working; following clinical policies; strong IT and clear telephone manner.
* Desirable: experience with audit and clinical risk management.


Other requirements

* Flexibility to work outside core hours; DBS check; Occupational Health clearance; nurse revalidation; CPD evidence.


Qualifications

* Essential: Registered Nurse (NMC).
* Desirable: mentor/teaching qualification; postgraduate qualification in Chronic Disease Management; family planning qualification.
