Offer description

Overview

Children and Young People (CYP) Mental Health Service (CYPMHS) South team located in Borehamwood and Watford is looking for an experienced nurse with experience of working with CYP service users at various phases of their mental health. Passionate about making a difference for individuals and their carer’s during their personal journey. Ambitious to work within this challenging role, in a challenging environment, with real scope to develop and influence practice. The post holder does not carry any care coordinator to a caseload of service users, but requires you to ensure the complex clinical pathways care plan is met and updated at all times by staff colleagues. To work as a member of the multi-disciplinary team.

As a staff member you are representatives of Hertfordshire Partnership NHS Foundation Trust (HPFT) and play a vital role in fostering positive relationships and communication between the Mental Health Trust and not only the service users and their carers and relatives, but also other agencies, charities and providers.

Providing urgent assessments to those considered to have acute mental health concerns and offer robust home treatment options as an alternative to clinic appointments where necessary.

A role that is varied and challenging, offering an opportunity to work in a variety of settings, with people with a variety of mental health needs. Additionally, you will work as part of a multi-disciplinary team to provide safe and effective treatment in the community, and facilitate discharges from the inpatient units to the community team and furthermore ensure discharges to primary care.

Working collaboratively with our service users, their carers and our colleagues to positively manage risk in the CYP care pathway, whilst enabling and empowering an individual’s recovery journey.

Demonstrate a high standard of record keeping and documentation with adherence to Trust policies.

Deliver care that is person centred reflecting current best practice, and challenges practice that may be detrimental to older people receiving high quality care.

Demonstrate a working understanding of the legal and ethical issues in providing mental health care, in particular having a good knowledge of the Mental Health Act (1983) and its use in non-mental health areas.

Hertfordshire Partnership University NHS Foundation Trust (HPFT) is an outstanding organisation with ambitions to match. We are one of just five mental health trusts to achieve an overall rating of ‘Outstanding’ from the Care Quality Commission, and our aim is to be the leading provider of mental health and specialist learning disability services in the country.

Our family of over 4500 members of staff provide health and social care for over 400,000 people with mental ill health, physical ill health and learning disabilities across Hertfordshire, Buckinghamshire, and Norfolk, delivering these services within the community and several inpatient settings. We also deliver a range of nationally commissioned specialist services including Tier 4 services for children and young people, perinatal services, plus medium and low secure learning disabilities services.

Our Trust Values Are

Welcoming. Kind. Positive. Respectful. Professional. These values are at the core of who we are, everything we do, and how we do it!

Would you like to be part of the HPFT family? Would you like to work with us to ensure our service users live the fullest lives possible they can? Would you like to be supported in your career to be the best that you can be?
