An exciting opportunity has arisen within Strathmore Day Opportunities.
We are looking for a dynamic individual who is motivated to support Older Adults living with a range of conditions and life experiences to remain active and included in their communities.
As a Social Care Officer, you will be responsible for undertaking link worker duties which includes file maintenance, co-ordinating and chairing reviews, formulating and developing personal plans to meet specified outcomes.
Strathmore Day Opportunities provides a flexible environment where Older Adults can come and enjoy stimulating activities while being supported by skilled and experienced staff.
You should be able to support people with a range of abilities to participate in the popular activities on offer e.g. artwork, cooking, gardening, singing to name but a few.
You will be part of a multi skilled team, primarily supporting older adults and may be required to provide support within our learning disabilities service.
You will be committed to providing person centred care within our Jessie street base and also opportunities within the wider community – this supports service users to meet their individual outcomes in line with a personal plan.
Duties will also include assisting individuals with personal care which can include intimate personal care, cleaning tasks within the building and other duties as required.
You will promote independence, choice, social inclusion, and well-being. You must be a team worker, flexible and able to cope positively with continuous change.
Strathmore Day Opportunities currently operates between 08:45 -16:00 Monday to Thursday and 08:45-15:45 on a Friday but this may be subject to change.
Skills & Experience Required
You will be required to work flexibly and carry out escort / driving duties on our daily transport requirement.
You must have a valid UK driving licence and a willingness to drive Perth and Kinross Council vehicles as this post requires you to provide transport for service users to a variety of locations across Perth and Kinross.
You should have experience of working with vulnerable adults in a community or residential setting.
Having SVQ 2 / 3 in Health and Social Care Adults is desirable for the post
You will have the ability to make decisions and know when to seek further advise from management.
You weill be able to develop good working relationships with colleagues, carers and other professional agencies to ensure outcomes are achieved.
The successful candidate should have excellent communication skills - verbal, non-verbal and written.
You will have good IT skills (Particularly Microsoft office applications), the ability to work on your own initiative and as part of a team.
You will take responsibility for yourself, customers/service users, contractors and others and ensure compliance with all relevant Health and Safety legislation.
You will promote and act as a role model for the Council’s values / behaviours framework and commitment to equality and diversity.
You will promote positive values and anti-discriminatory, non-judgmental practice and treat colleagues and customers with dignity and respect in line with the Employee Code of Conduct and SSSC Codes of Practice.
