Strategic Manager Integrated Neighbourhood Team

This Job will be hosted by Three Harbours and Bosvena PCN on behalf of the Integrated Neighbourhood Team, working collaboratively with all system partners.

Provisional interview date is 5th August 2025

Job Purpose

The INT Strategic Manager will lead the development,implementation, and evolution of the Integrated Neighbourhood Team (INT) model,driving the transition from design and mobilisation to full operationaldelivery. Working closely with system partners the postholder will ensure the INT delivers integrated,person-centred care, reduces health inequalities, and improves populationhealth outcomes.

This is a dual-faceted role: in the initial stages, thefocus will be on change management and programme leadership to establish theINT, with a gradual shift towards overseeing day-to-day operational deliveryand continuous improvement.

The initial primary piece of work will be a changemanagement programme to align key service providers within the INT to become acohesive and high-functioning frailty team. You will bring together workforcefrom primary and community teams, and work to support the Clinical Leadershipfrom the Frailty Lead GP linking with hospital Eldercare Consultants to createa hospital in-reach and pull service to ensure that people return to their ownbed as soon as possible. This will be a wide-MDT approach working with local organisations, who may support people within our community.


Main duties of the job

Key Responsibilities

1. Strategic Leadership & Change Management

* Adopt and apply the key principle of the NHS 10-year plan to working within community at place and lead on integration and shared goals.
* Demonstrate a sound knowledge of the INT population drawing on themes from population health data being curious to extract reports, data and analytics that can be shared with wider system partners and commissioners.
* Lead the co-design and development of the INT model, working to strengthen a leadership team with system partners and local communities.
* Act as the key point of coordination for all INT development activities, facilitating collaboration across organisations and ensuring alignment with ICS strategic priorities.

2. Service Transformation & Pathway Design

* Lead on a service redesign around frailty to bring together primary, community, social care and secondary care teams to wrap a team around this cohort of patients and provide cohesive and appropriate care at place.
* Map existing services and resources across the neighbourhood, identifying gaps, duplication, and opportunities for integration.

3. Operational Oversight & Performance Lead

* As the INT matures, assess and evaluate the needs or the INT to ensure a transition to operational leadership, overseeing day-to-day service delivery and workforce management.
* Establish and monitor performance dashboards, using key metrics to measure impact, track progress, and inform continuous improvement.


About us

This role will be based in the Three Harbours and Bosvena Integrated Neighbourhood team area, and will work with primary care services that cover the patient footprint for Bosvena Health, Fowey River Practice, Lostwithiel Medical Practice and Middleway Surgery - approximately 43,500 people live within this catchment area, and will be served by the INT.

The role is a senior leadership position working collaboratively within the INT steering group which brings together commissioners from the Integrated Care Board, General Practice Clinical Leadership, Adult Social care, Community Care (Cornwall Foundation Trust), Voluntary Sector organisations, and all health and wellbeing services based in this community.

The role will work peripatetically within the neighbourhood linking teams and forging strong links with secondary care colleagues to provide joined-up healthcare at place. You will join a dedicated and caring team of professionals that all want to improve health outcomes for our population, reducing health inequalities and providing a protecting, caring and preventative services for our population.

* Develop and deliver robust implementation plans for all key project areas, including clear milestones, success metrics, and risk management processes.
* Act as the key point of coordination for all INT development activities, facilitating collaboration across organisations and ensuring alignment with ICS strategic priorities.
* Foster a culture of shared leadership and collective accountability, to support clinical and professional leaders to shape and drive service transformation.
* Manage stakeholder engagement, building trusted relationships with local practices, local authorities, VCSE organisations, and patient groups.

2. Service Transformation & Pathway Development

* Lead on a service redesign around frailty to bring together primary, community, social care and secondary care teams to wrap a team around this cohort of patients and provide cohesive and appropriate care at place.
* Work with Third Sector colleagues to assist in supporting the INT and community to maximise their benefits to the INT
* Map existing services and resources across the neighbourhood, identifying gaps, duplication, and opportunities for integration.
* Continue to facilitate the redesign of pathways to enable more proactive, preventative care, keeping people well in their communities and avoiding unnecessary hospital admissions.
* Support the adoption of population health management approaches, using data to identify at-risk cohorts and design targeted interventions wherever possible.
* Champion digital innovation, exploring technologies that can enhance connectivity, care coordination, and patient outcomes.
* Consider opportunities for joint estate usage within the INT or wider ICA footprint.
* Lead on creation of single INT leadership team

3. Operational Oversight & Performance Management

* As the INT matures, assess and evaluate the needs or the INT to ensure a transition to operational leadership, overseeing day-to-day service delivery and workforce management.
* Establish and monitor performance dashboards, using key metrics to measure impact, track progress, and inform continuous improvement.
* Develop and oversee governance arrangements, ensuring robust decision-making, financial stewardship, and regulatory compliance.
* Create a financial risk profile for the INT and manage and report on financial performance.
* Lead on workforce planning, supporting new roles, shared staffing models, and multi-agency working to build a sustainable, resilient team.

4. Continuous Improvement & Embedding a Learning Culture

* Foster a learning culture within the INT, embedding quality improvement methodologies and promoting reflective practice.
* Facilitate peer learning and shared development opportunities across neighbourhood teams.
* Develop strategies to collect and analyse patient and community feedback to refine and evolve service delivery, ensuring care is responsive to local needs and preferences.
* Share insights and best practices with wider system partners, contributing to the ICSs evolving neighbourhood care model.

Person Specification

Essential Qualifications & Experience

* Proven experience in change management, service transformation, or programme leadership within health or social care settings.
* Track record of delivering complex, multi-agency projects, ideally involving integrated or community-based care models.
* Strong understanding of population health management, health inequalities, and the wider determinants of health.
* Experience of operational service management, including performance monitoring, governance, and workforce leadership.
* Highly developed relationship-building, facilitation, and negotiation skills, with the ability to navigate organisational complexity and influence at all levels.
* Experience working within or alongside primary care, with an understanding of PCNs, general practice, and community service delivery.
* Educated to Masters level or evidence of equivalent study or learning.

* Educated to Masters Level
* Diploma or similar in primary care management
* Project Management qualification or experience in service redesign
