UK CONTRACT OR SELF‑EMPLOYMENT
Start date & Duration: 9/12/24 ‑ 31/12/24 (3 weeks inclusive) and then again 21/1/25 ‑ 11/2/25
Location: Frimley near Camberley, Surrey. A small village with plenty of shops, surrounded by green space and countryside.
Patient: Male – 22/10/58 age 66
General description of patient’s health:
Mr has Primary Progressive Multiple Sclerosis. He experiences muscle spasms, stiffness, weakness, immobility, bladder problems, pain and fatigue. Because of reduced upper‑body strength he cannot use his lower body and requires regular repositioning in his recliner chair and bed. He becomes stiff and painful at night and needs repositioning twice during the night and once in the early morning. This is assisted by an additional overnight domiciliary carer.
General information about Mr:
Values: family, wife (who lives with him), children and grandchildren. He wishes to remain living at home and values support that includes a good sense of humour. He has a small terrier, Monty. He enjoys speaking with family on the phone, being with his wife and dog.
Care needs
Full personal care support: he cannot move any part of his body independently. Provide assistance with eating, drinking and all personal care. Assist with trips to the bathroom or commode using the ceiling hoist. Apply creams if prescribed to maintain good skin integrity and reduce risk of breakdown. Change pads as required. Dispense and administer medication at the times required. Provide cough‑assist machine and nebuliser as needed.
Equipment

* Oxygen cylinder used every night and during the day as required
* All personal care is carried out in bed
* Motorised wheelchair ramp into car – careful attention needed
* Live‑in carer will drive patient to the gym twice a week for physiotherapy and attend routine doctors’ appointments
Diet
* Patient is prone to choking and has been advised to have a pureed diet, but chooses a normal diet
* Assistance with all feeding and meal preparation
* Support with drinks throughout the day
Household duties
* General tidying (wife does most of it)
* Hoover and polish regularly
* Keep bathroom and bedroom clean
* Load and unload dishwasher, put culinary items away
* Clean microwave after use
Risks
Use of electric wheelchair ramp into car – requires careful attention.
Pets
Dog – a small friendly terrier called Monty.
Language
A good understanding of both conversational and written English.
CAR
Automatic – the caregiver will be insured by client.
FOOD
Full access to kitchen – agency provides a £50 food allowance per day.
