Overview

We are looking for an enthusiastic and experienced Band 6 Staff Nurse or Therapist with specialist knowledge in discharge planning and pathways to join our Community Transfer of Care Hub at Gravesham Community Hospital.

In the Community Transfer of Care Hub you will manage all referrals into the Discharge to Assess Pathways across Dartford, Gravesend and Swanley, working with a multidisciplinary team of integrated discharge nurses, therapists, rehabilitation and healthcare assistants and an administrator to assist in discharge planning across acute hospital services.

Gravesham Community Hospital is located close to St Georges Shopping Centre with nearby dining options, making it an ideal base.

Although based at Gravesham Community Hospital, you will travel to local acute hospitals to provide in-reach support and visit patients at home and in care homes to undertake assessments in line with our "Home First" approach. A driving licence and access to a car for work purposes are essential for this role.


Main duties of the job

As a Band 6 Team Sister / Therapist you will:

* Provide clinical/management leadership and day-to-day support for the Community Transfer of Care Hub Team.
* Support the Team Senior Sister and Clinical Service Manager with recruitment as required.
* Participate in induction of new staff, students, bank and agency staff.
* Involve patients and carers in the planning and delivery of community services.
* Maximise use of clinical resources and organise clinical cover as needed.
* Monitor and evaluate patient flow, admissions and discharge arrangements; initiate improvements.
* Support safe and timely discharge planning for team members.
* Take responsibility for the most complex cases, including leading on the assessment of homeless patients to ensure statutory responsibilities are met.
* Review complex patients requiring 1-1 funding and support safeguarding issues as needed.
* Develop and support robust communication with HCRG Care Group colleagues and other agencies (GPs, social care, nursing colleagues).
* Take clinical responsibility for placements made by the Community Transfer of Care Hub and address any arising issues.

Job responsibilities

The Ideal Candidate

Our ideal candidate will be / will have:

* Registered Nurse (RN) or registered Therapists (OT / Physio) with current live PIN on the NMC or HCPC register
* Recognised teaching/assessing qualification 998/997/mentorship
* Continuous Professional Development Portfolio
* Specialist knowledge and experience in discharge planning and pathways
* Experience of working within a multiagency health and care system
* Relevant post-registration clinical experience

As travel is required, a driving license and access to a car is essential.
