Offer description

1. Deliver Person-Centred Support Support delivery of the outreach service for the Rough Sleepers Project, providing tailored mental health and wellbeing support. Arrange and deliver 1:1 wellbeing appointments for individuals accessing the service. Co-produce personalised wellbeing plans, rooted in the 5 Ways to Wellbeing, based on each individuals strengths, priorities and identified needs.

Monitor and review wellbeing plans, offering additional guidance and adapting actions where needed. Promote resilience and recovery by supporting individuals to develop and maintain their own recovery plans. Contribute to wider organisational priorities and undertake additional duties across Swindon & Gloucestershire Mind as required. 2.

Effective Collaboration with External Agencies Work closely with care coordinators and professionals within the NHS Swindon Recovery Team. Operate as part of a multi-agency team across sites including The Haven and Booth House. Attend monthly Rough Sleeper Panel meetings and other relevant partnership meetings. Participate in referral and handover meetings to ensure smooth transitions and continuity of care.

Signpost and support individuals to access appropriate internal and external services, resources and up-to-date information. 3. Development and Promotion of the Rough Sleeper Service Lead and facilitate the Rough Sleeper drop in clinics, actively promoting sessions and encouraging engagement. Maintain a visible presence at partner sites, strengthening relationships and increasing awareness of the service.

1. KPIs and Data Excellence Accurately record, evidence and report outcomes in line with organisational standards. Meet agreed monthly targets relating to wellbeing plans and reviews. Capture, track and monitor performance using internal systems to ensure quality and accountability.

General Responsibilities Maintain strict confidentiality and accurate record-keeping in line with data protection and GDPR requirements. Contribute to effective system management, ensuring compliance with organisational policies and procedures. Undertake administrative duties, collecting and reporting data to demonstrate impact and outcomes. Adhere to legal, regulatory and health and safety requirements at all times.

Build positive relationships with statutory and third-sector partners to strengthen collaboration and service reach. Keep up to date with relevant legislation, policies and quality standards, ensuring practice remains compliant and evidence-based. Contribute positively to team culture through active participation in meetings, supervision, appraisal and ongoing professional development. Work collaboratively with managers and senior leaders to support continuous improvement and organisational success.

Champion the 5 Ways to Wellbeing model within all aspects of support delivery. Undertake risk assessments using the UK Mental Health Triage Scale (UK MHTS) to ensure safe, appropriate and responsive support.
