Band 6 Cardiac Rehabilitation Clinical Nurse Specialist (CNS)

An exciting opportunity has arisen for a motivated nurse to join our friendly, driven, passionate and nationally accredited Cardiac Rehabilitation services. The cardiac rehabilitation team provides a vital service to our patients who have suffered Acute Coronary Syndrome (STEMI & NSTEMI) and those post coronary revascularisation (PCI and CABG surgery).
This post is for a qualified Nurse working 23 hours per week over three days, based at Good Hope Hospital, but may also have to cover other hospital sites (Birmingham Heartlands and Solihull Hospitals) as required.
This post will suit a dynamic and enthusiastic candidate who has a positive outlook and is able to motivate patients to make changes. Experience of working in an exercise environment and/or exercising yourself will also be beneficial.
Main duties of the job
You will provide education to patients and their families before discharge, develop individualized pharmacological and non-pharmacological plans for their Cardiac Rehab and communicate this effectively with primary care, to ensure a seamless service is offered to patients. Under the guidance of experienced Clinical Exercise Physiologists you will also support patients in the outpatient Cardiac Rehabilitation programme providing health education and exercise supervision and guidance both remotely and face-to-face. This includes assessing patients' suitability and risk stratification for core rehab (phase 3) and referring onto the most appropriate programme depending on patients' risk and choice in conjunction with the multidisciplinary team.

Qualifications
* Evidence of post registration CPD, post registration qualification in cardiology and professional knowledge supplemented by specialist experience and short courses to degree / masters level equivalent FHEQ Level 6 /7.
* Recognised mentorship course.
* A willingness to undertake training and education to support the role of Non‑medical prescriber.
Experience
* Evidence and ability to revalidate as required by the NMC.
* Significant post registration experience with evidence of passion/enthusiasm and knowledge for Cardiac Rehabilitation.
* Evidence of achievements and career development in current post and ongoing professional development/competence.
* Demonstrate specialist knowledge underpinned by theory and CPD relevant to speciality.
* Experience and enthusiasm for teaching/assessing and supervising others in a clinical setting.
* Experience of working in a multi‑disciplinary and cross agency environment.
Additional Criteria
* Knowledge of current relevant NHS policy context relating to the speciality.
* Understanding of safeguarding principles in relation to the role.
* Knowledge of corporate and clinical governance.
* Ability to adapt behaviour to changing circumstances.
* Positive and enthusiastic attitude.
* Ability to communicate articulately and motivate others.
* Ability to work a varied shift pattern and across UHB sites.
* Ability to act on own initiative.
* Awareness of current research related to the speciality.
