Overview
The successful candidate will be a Registered Nurse from either Acute & Community or Mental Health background who is keen to develop their assessment skills further in working in an autonomous role within the NHS Long Term Care Service. This role emphasizes multi-disciplinary working and requires a clear understanding of Continuing NHS Healthcare & NHS Funded Nursing Care policies, the related financial implications, and the Social Services and Well-being (Wales) Act 2014. The post is with Hywel Dda University Health Board, a dynamic and innovative organization that has invested in the NHS Long Term Care Service and implemented a new model focused on person-centred care. A new Long Term Care Pathway supports secondary care with the Continuing NHS Healthcare and NHS Funded Nursing Care processes to ensure a smooth transition from acute episode through discharge planning to Nursing Home placement.
The base can be in Carmarthen, Llanelli or Pembroke Dock, but may be subject to change. You will need to travel across the Hywel Dda University Health Board area as well as neighbouring counties. The ability to speak Welsh is desirable; English and/or Welsh speakers are equally welcome to apply.
Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. We have over 13,000 staff and together provide primary, community, in-hospital, mental health and learning disability services. We work in partnership with local authorities and public, private and third-sector colleagues, including volunteers. Our services are provided in:
* Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
* Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
Responsibilities
* Assess and monitor individuals in the Nursing Home sector and provide training and support to Nursing Homes.
* Review and assess patients for Continuing NHS Healthcare and Funded Nursing Care.
* Manage MDT meetings related to funding and discharge planning.
* Work within the Long-Term Care Pathway to support transitions from acute care to discharge planning and Nursing Home placement.
* Ensure identified care needs are met, funding is appropriate, and monitor standards of care within Nursing Homes.
* Work autonomously as well as part of a multi-disciplinary team and travel across the Hywel Dda UHB area.

* Registered Nurse from Acute & Community or Mental Health background
* Excellent assessment and communication skills
* Ability to work both as part of a team and autonomously
* Desirable: ability to speak Welsh; English and/or Welsh speakers welcome
