Health & Care Programme & Development Manager (2 posts)
Salary: £51,801 - £59,724 per annum
Grade: G
Fixed Term Contract - 2 years
37 Hours per Week
The role is responsible for the development and implementation of the Adult Social Care transformation programme - as part of Better Care Together Thurrock and the Integrated Care Strategy. Although the role will support a range of transformation programmes, there will be a focus on the development and implementation of our Integrated Locality Teams.
The role sits within Adult Social Care but will work in partnership with Housing and local NHS and voluntary and community sector partners.
The role will support the development and implementation of our approach to place-based working and our ambition to improve the outcomes of local people through the development of integrated and person-focused solutions and system-change.
Experience and skills include:
* A good understanding of health and social care.
* A good understanding of the role of the community and voluntary sector and asset-based community development.
* Experience of delivering change initiatives and/or system redesign to improve health and care outcomes.
* The ability to and experience of working in partnership with others to develop and deliver change.
* A Standard Disclosure and Barring Service (DBS) check is required.
