Outreach Caseworker
Location: Covering Preston Park, Coldean, Moulsecombe, Portslade, Hove, Rottingdean, Lewes
Salary: £30,000 per annum
Hours: 35 per week
Contract: Fixed term contract to September 2027
Additional Requirement: Applicants must have a full licence (2 years) and access to a vehicle.

You'll help them identify unmet needs, build coping strategies, and connect with the right support. This role is vital in creating a more compassionate, efficient, and person-centred care environment.
What will a day in the life of an Outreach Caseworker involve?
1. Supporting individuals with complex needs to reduce their use of A&E, 999, and mental health services.
2. Building strong relationships with health, social care, and emergency service partners.
