Support Coordinator. You will be at the heart of supporting customers with housing needs to develop their skills to live more independently in the community.
What you’ll do
* Update support plans with your customers and coordinate with the Lead Support Coordinator.
* Work with people aged 18+ who may have experienced homelessness.
* Help customers with daily living activities, including making meals, shopping, budgeting, maintaining tenancy, and moving onto independent accommodation when appropriate.
* Carry out risk assessments, support planning, goal setting, and regular reviews.
* Maintain regular communication through meetings, chats, and catch‑ups.
* Co‑operate with colleagues, with ample time for collaboration.

You have
* Passion to support our customers to live their best life, working collaboratively with an eye for detail.
* Experience or knowledge of creating person‑centred support plans and supporting colleagues in working to those plans.
* Experience of coordinating and assessing customer referrals.
* Ability to work on your own initiative, remain calm under pressure and show resilience.
The practical bits

* Flexibility may be required to meet customers’ needs, such as adapting start or finish times.
* You’ll need to use technology for creating and updating support plans, completing online learning and collaborating with colleagues.
* Enhanced DBS check is required and will be paid for by us.
