Job Title: Team Manager - Transition Team - INTERNAL APPLICANTS ONLY

Hours: Full Time, 37 hours per week
Location: Neath Civic Centre, Water Street, Neath. SA11 3QZ
Workstyle: Hybrid Working
Salary: £50,754 - £53,788 (including £3,000 market pay supplement pro-rata). This payment is reviewed on a 12 monthly basis.

The Community Network Team (Transition), known as the Transition Team, is seeking a dedicated and experienced Manager to lead our new Transition Team and deliver on Neath Port Talbot’s transformation agenda. As the Transition Team Manager, you will play a pivotal role in ensuring the delivery of high-quality services to young people preparing for adulthood and transitioning to adult services across Neath Port Talbot.
Key Responsibilities:
* Lead the strategic direction and vision of the Transition Team.
* Ensure seamless transition from children's to adult services.
* Coordinate support for young people and their families/carers during the transition period.
* Prepare young people for adulthood, including education, employment, and independent living.
* Working in partnership with health, social care, education, and community resources.
* Embed best practices and foster a positive work environment and team culture.

Requirements:
* Social work qualification and professional registration with Social Care Wales.
* In-depth knowledge and working practice of the Social Services and Well-being (Wales) Act 2014.
* Familiarity with Continuing Health Care, Mental Capacity Act 2005, All Wales Safeguarding Procedures, and Deprivation of Liberty Safeguards.
