Job overview We have a band 3 Community Support Worker position in our Community Response Team based at Walton Hospital, Chesterfield. Our team provides vital care and support to patients in their own homes, particularly following a crisis or after discharge from hospital. This service ensures that individuals receive the appropriate care and assistance they need to recover and maintain their independence in a familiar environment. We work collaboratively across a large geographical area and maintain close links with our other planned and rapid teams in North East Derbyshire and Chesterfield to deliver seamless, patient-centred care. Main duties of the job To provide holistic care to patients in their own homes, under the supervision of the appropriate registered professional. To communicate information effectively To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members To maintain and record accurate health records electronically, in line with DCHS policies. To participate as a team member in a wider integrated team, in continuous learning to develop your own knowledge and skills To have a level of understanding with regards to the importance of safeguarding. Respect the dignity of individuals. Obtain consent when involving the patient, their carers/families/relatives in shared decision making. Person specification Education, Qualifications and Training Essential criteria NVQ Level III in care / BTEC Health & Social Care or equivalent Experience of working health care setting Experience and Knowledge Required Essential criteria Understanding of factors that influence health and wellbeing Understanding of the implications for, and responsibilities of the post for risk management and safeguarding (2) Desirable criteria Experience of working with Palliative Care Experience of working in a rehab setting Experience of working with the Elderly Skills and Attributes Essential criteria Good verbal communication skills Well-developed inter-personal skills Able to keep accurate records Desirable criteria Previous use or knowledge of SystmOne IT Skills Ability to manage own time and prioritise workload Other Essential criteria Able to travel across the county to attend essential training and other required training for your role Full UK driving licence with access to a vehicle Derbyshire Community Health Services NHS Foundation Trust (DCHS) is one of the largest providers of specialist community healthcare in England and we currently employ around 4,500 staff.
