An excellent opportunity has arisen for an enthusiastic, experienced Care Navigator to join our friendly practice team with 5 partners and 8,400 patients. The post is for 17.5 hours per week, with a requirement to remain flexible to cover colleagues who may be sick or on holiday.
Main duties of the job
To undertake a wide range of reception, secretarial, and administrative duties, providing support to the multidisciplinary team. Duties include greeting and directing patients, managing appointment systems, booking appointments, processing information (electronic and hard copy), and assisting patients. The role acts as the central contact point for patients, liaising with clinical team members and external agencies such as secondary care and community services, following current policies, including the use of the electronic referral service (ERS).

Job responsibilities
* Booking appointments and recalls per practice policy
* Familiarity with all appointment systems, including variations
* Processing personal, telephone, and e-requests for appointments
* Signposting patients to appropriate services
* Routing patients correctly during appointments
* Managing urgent consultations efficiently
* Responding to queries from patients and visitors
* Signing in visitors and informing staff
* Explaining practice procedures to new and temporary patients
* Data entry for new and temporary registrations
* Logging home visit requests and referring as needed
* Acting on lab results according to protocol
* Handling incoming calls and messages
* Communicating with patients and external agencies
* Passing unresolved issues to the next shift
