Lincolnshire Community Health Service (LCHS) invites applications for a 0.6 WTE fixed‑term contract (6 months, 3 days a week) for Registered Nurses to join the Lincoln North Community Nursing Team.
Key Responsibilities
* Deliver nursing care autonomously and as part of a multidisciplinary team in patients' homes, care homes and clinic settings.
* Assess patients' care needs, plan, provide and evaluate nursing care using evidence‑based practice.
* Provide key interventions such as wound management, continence care, palliative care, medication administration and long‑term condition management.
* Collaborate with the MDT (Social Workers, GPs, Allied Health Professionals) to ensure coordinated care delivery.
Working Conditions
Service hours are 08:00‑18:30 hrs, seven days a week. Variable shift patterns include early shifts (08:00‑16:00) and late shifts (10:30‑18:30). Weekend shifts are 08:00‑18:30. A 12‑week induction programme and ongoing preceptorship and clinical skills training are provided.
Requirements
* Registered Nurse (RN) with current registration in the UK.
* Ability to work independently and in a team.
* Access to reliable transport to reach patients' homes and clinic sites, particularly in rural areas.
* Willingness to work a flexible schedule and manage variable shift patterns.
