Responsibilities
* Provide comprehensive information, advice, and community social work within the Community Response and Reablement Team, assisting new individuals in the community.
* Work within the Community Independence and Wellbeing Team to support existing residents with their social work and occupational therapy needs.
* Deliver excellent social work and social care support with the Hospital Team, ensuring effective discharge planning for individuals, carers, and families.
* Collaborate with Community Teams dedicated to people with learning disabilities and mental health needs to deliver tailored support that enhances quality of life.
