Responsibilities Deliver comprehensive advanced-level patient care by independently diagnosing, planning, and managing treatment for a wide range of conditions within the scope of your professional practice. Conduct thorough patient assessments, including interpreting diagnostic tests and formulating treatment plans tailored to individual needs. Provide expert clinical advice and leadership to patients, their families, and the wider multidisciplinary team. Collaborate with colleagues across disciplines to promote integrated and holistic care for patients. Actively contribute to the development and implementation of evidence-based protocols, standards of care, and guidelines. Participate in clinical audits, quality improvement initiatives, and research to ensure continuous improvement in service delivery. Mentor and support junior staff, nursing teams, and students to foster skill development and professional excellence. Ensure all clinical decisions and documentation meet legal, ethical, and professional standards. Qualifications Master’s degree in Advanced Clinical Practice (or equivalent qualification). Valid and unrestricted Registered Nurse (RN) license. Advanced Practice certification, with evidence of prescribing qualification or eligibility. Demonstrable experience working as an Advanced Clinical Practitioner or in a similar advanced nursing role. Proven ability to manage acutely ill patients, including knowledge of chronic conditions, pharmacology, and diagnostic reasoning. Strong communication and interpersonal skills, with the ability to effectively engage and empower patients and multidisciplinary teams. Commitment to evidence-based practice and ongoing professional development. Understanding of the importance of equality, diversity, and inclusion in healthcare settings. Day-to-Day Start the day by reviewing patient case notes and preparing for scheduled patient consultations. Conduct patient assessments, including history-taking, physical examinations, and reviewing diagnostic results. Develop and implement treatment plans, liaising with the broader care team to ensure a multidisciplinary approach. Spend time mentoring and supporting junior staff and peers, sharing your expertise and providing guidance. Lead discussions during patient rounds or team meetings to create cohesive care strategies. Engage in clinical audits, review patient outcomes, and collaborate on strategies for continuous service improvement. Document all patient interactions accurately and maintain patient confidentiality, adhering to ethical practice standards. Build meaningful relationships with patients from diverse backgrounds, ensuring all communications are respectful and inclusive.
