About the role

A day in the life of a Support Worker includes:
1. Taking patients home from hospital.
2. Supporting individuals to ensure basic resettlement on their return home, including food, water, and heat.
3. Building positive relationships with service users, their families, and other healthcare professionals.
4. Ensuring all care documentation is accurate and up to date.
Requirements to be a successful Support Worker:
1. Previous experience in a similar care setting, preferably healthcare or social care.
2. Compassionate and empathetic nature, with a strong desire to make a positive impact.
3. Excellent communication and interpersonal skills, effective within a multidisciplinary team.
4. Knowledge of person-centred care principles, upholding dignity and autonomy.
5. Ability to work independently, demonstrating initiative and reliability.
6. Flexibility to work various shifts, including evenings, weekends, and bank holidays.
7. A full UK driving license is required.
