The PCN Pharmacy Technician will support medicines optimisation andsafe, effective prescribing across five GP practices within the PCN. The postholder will work as part of the multidisciplinary primary care team, supportingGPs, pharmacists, nurses, and other healthcare professionals to improve patientoutcomes, reduce medicines-related risk, and contribute to efficient practicesystems.
Main duties of the job
* Support repeat prescribing processes in line with local and national guidance
* Carry out medication reviews under the supervision of a clinical pharmacist or GP (where appropriate)
* Identify and resolve medicines-related issues, including:
* Over- or under-ordering
* Synchronisation of repeat medicines
* Medicines adherence concerns
* Support medicines reconciliation following hospital discharge and outpatient clinic letters
* Review and action prescribing tasks, queries, and workflows
* Support cost-effective prescribing and medicines optimisation initiative.
* Respond to medicines-related patient queries (telephone, online, or face-to‑face)
* Support structured medication reviews (SMRs) by gathering relevant information
* Support long‑term condition management through medicines optimisation
* Promote safe use of medicines and provide basic medicines advice within scope of practice
* Support clinical audits related to prescribing and medicines safety
* Identify and escalation medicines safety concerns, including high‑risk medicines
* Support compliance with local, PCN, and national prescribing policies
* Contribute to meeting PCN and practice QOF, IIF, and contractual requirements
* Maintain accurate and timely documentation within patient records

Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.
Qualifications
* Experience working in primary care, general practice, or community pharmacy
* Good understanding of repeat prescribing systems
* Knowledge of medicines optimisation and prescribing safety
* Ability to work independently and as part of a multidisciplinary team
* Excellent communication and organisational skills
* Previous experience working in General Practice or PCN
* Experience with medication reviews and medicines reconciliation
* Understanding of QOF IIF and PCN contractual requirements
* Experience working across multiple sites
