At Alzheimer’s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking to welcome an approachable, enthusiastic, and caring Dementia Adviser to join our supportive team in Coventry area working 35 hours per week.
This is a community-based role involving regular travel across the Coventry area including local home visits, meetings and supporting and facilitating a range of society groups, including the delivery of awareness raising talks and dementia friends’ sessions with the flexibility to work from home based on business and community needs.
You will work closely with people living with dementia, carers, their family members and friends providing ongoing support. You will listen carefully, understand what matters most to each person and offer person-centred support and guidance that helps them to feel empowered and enable them to access vital services.
There is an emphasis on partnership working and networking with Health and Social Care professionals, including GPs, Social Prescribers, Memory Assessment Services and Social Services. You will need experience in networking effectively and collaborating with other professionals to achieve a positive outcome for service users.
Your days will be varied and meaningful. You might be visiting someone at home, offering calm reassurance over the phone, or working closely with other professionals to make sure the right support is in place. You will manage your own caseload, keeping clear and accurate records.
You will be part of a pro‑active, caring and supportive local team who share ideas, reflect together and support one another.
Interviews for this role have been provisionally scheduled to take place face to face at the Community Dementia partnership Hub, Everdon road Coventry CV64DT on w/c 6th July.
