In March 2023, St Elizabeth Hospicemerged with East Coast Hospice to form St Elizabeth East Coast Hospice, withall service provision in Great Yarmouth and Waveney coming under the StElizabeth East Coast Hospice name.
This is an exciting time for StElizabeth East Coast Hospice, as we are developing an achievable andsustainable palliative and end of life care service for the residents of GreatYarmouth and Waveney. Its a time of expansion, development and investment inthe Hospice Team. We are offering an opportunity to be part of the CommunitySpecialist Palliative Care Team and also work within our Community Care Unit influencingthe holistic care of patients and carers with end of life and specialistpalliative care needs. Main duties of the job
We arelooking for an experienced Registered Nurse to join our Community Team. You will work with our multi-disciplinaryteam providing specialist palliative care advice and support to patients in theGreat Yarmouth and Waveney area in the community and at one of our CommunityCare Units.
St Elizabeth Hospice continues to driveforward innovation and development of services, both in the Ipswich and EastSuffolk locality and further up the coast to the Great Yarmouth and Waveneyarea. In 2019 NHS Norfolk and Waveney IntegratedCare Board awarded the specialist palliative and end of life care contract toEast Coast Community Healthcare. The contract includes a specialistpalliative care service which will be delivered in partnership with StElizabeth Hospice. Our most recentCare Quality Commission report rated the hospice as Outstanding.

Job responsibilities
JOBSUMMARY
To work at anenhanced level of practice to manage community patients in complex, challengingand changing situations and environments, seeking further guidance when theboundary of competence is reached. Willpossess skills and capabilities across all four pillars of nursing: clinicalpractice, education, research and leadership. Required to function to a highlevel of autonomy within specialist palliative care.
KEYACCOUNTABILITIES
To undertake in-depthholistic assessments of patients with palliative care needs, regardless oftheir diagnosis and plan care to meet patient outcomes.
To provide information,advice, psychological and spiritual care to patients, their families andcarers, liaising and referring on as necessary to appropriate healthcareprofessionals in the Hospice, Hospital and community.
Responsiblefor and manage a caseload of patientsoffering continuity of care.
Work in acollaborative, compassionate way, promoting equality, respecting diversity, andensuring inclusivity, taking responsibility for challenging behaviour that doesnot align with these values.
Apply clinicalreasoning to individual presentations and complex situations, utilisinginformation from different sources to make timely, appropriate, andevidence-based judgements.
Optimise safety throughassessment and management of risk.
Act as a key role within the IntegratedNeighbourhood Team (INT) taking a population health approach to assist in themanagement of the end of life population within the INT.
Use advanced communication skills indelivering highly emotive information clearly, sensitively and unambiguouslywith staff, patients, families and professionals by face to face, telephoneand/or electronic means. This may bewhere there are barriers to understanding.
Provide skilled and expert palliativenursing interventions, working closely with medical and other colleagues toensure a coordinated and effective service across boundaries of inpatient,community and outpatients and across professionals.
Maintain and develop workingrelationships with community and hospital colleagues ensuring effectivecommunication so enhancing patient care
Involve patients,families, and carers in co-production of and making informed decisionsregarding strategies to manage their own health and wellbeing.
Participate in the developmentand updating of practice policies/guidelines and procedures using high-qualitycontemporary evidence-based resources.
Participate in the evaluationof significant events, capturing the learning from these experiences to improvepatient care and service delivery.
Identify own personaldevelopment needs, proactively seek feedback, and take effective action toaddress them using a mix of strategies.
Share knowledge,experience and expertise with nurses and others at all levels, throughstrategies such as formal and informal teaching, facilitated reflectivepractice, skills supervision, coaching, mentoring and preceptorship.
Participate in thesupervision, evaluation and assessment of nurses aspiring to work at anenhanced level.

Ensure adherence with relevant Infection Preventionand Control policies and procedures, raising any risks to the IPC group / servicelead.
Lead and participate in research,integrating research-based evidence in all aspects of specialist palliativecare into clinical practice
Contribute to the audit programme,taking a lead role in specialist palliative nursing audits.
Deputise for the Community Service Leadas requested.
Keep abreast of national policy onpalliative and end of life nursing strategy to use as a framework for practicedevelopment.
To update ownspecialist skills and knowledge in cancer and palliative care, throughcontinuous professional development

Because of the special nature of the Hospice and its work,the post holder may on occasion be asked to undertake other duties to helpmaintain our high standard of care.

Qualifications
* First level Registered Nurse with current NMC registration.
* Level 6 and / or level 7 post registration education in palliative care or related qualification.
* Advanced communication course.
* Non -Medical Prescribing.
* Counselling qualification.
Specific Skills
* Expert current knowledge of pain and symptom management, which underpins theory, usually gained through recent clinical practice at a senior level.
* Ability to identify psychological or spiritual distress requiring referral to other specialist services.
* Evidence of counselling skills and practical application.
* Evidence of ability to interact and influence at all levels within the organisation.
* Commitment to clinical excellence through regular professional updating.
* Computer literacy sufficient to input patient records and work efficiently and effectively.
* Able to prioritise and manage own time effectively.
Experience
* Significant post registration experience including proven palliative care / Oncology experience at a Band 6 or above, or equivalent level.
* Experienced in assessing patient needs that may be complex and subject to change.
* Knowledge of the national palliative care agenda.
* Have an understanding of safeguarding principles and be able to apply these within everyday practice.
* Experience of working in the community.
* Previous experience of developing and implementing policies and procedures.
