SIGN ON BONUS - FT $1,500, PT $750
Salary based on years experience (doesn’t include shift diffs) - $18.00 - $22.09/hr.

The primary purpose of this position is to provide quality nursing care to residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by your supervisor. Home health aide (HHA) or certified nursing assistant (CNA) required.
Responsibilities
* Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality.
* Demonstrates respect, attention and awareness toward the diversity among the people we serve (residents, families, caregivers, etc.).
* Is knowledgeable of the individualized care plan for residents and provides support to the resident in accordance with their care plan.
* Identifies and participates in QAPI process or area of development.
* Assists in daily requirements and tasks in care and treatment of residents, including bathroom assistance, personal hygiene, dressing, mobility, and food intake as per care requirements.
* Maintains resident rights and confidentiality.
* Documents all ADLs in electronic health record.
* Observes and reports symptoms, reactions and other changes in condition promptly.
* Routinely turns and positions residents as required by care plan.
* Reports incidents or evidence of resident abuse or violations of resident rights to the charge nurse or administration.
* Honors resident right to refuse care and reports such refusal to nurse supervisor.
* Maintains a safe, neat and clean environment.
* Answers resident call bells promptly and courteously.
* Checks residents who are unable to call for help.
* Takes and documents temperature, pulse and respirations, intake/output, and records in EHR.
* Ambulates and transfers residents using appropriate assistive devices and body mechanics.
* Operates only equipment trained to use and reports any defective equipment to supervisor.
* Communicates effectively and tactfully with residents, visitors, families, peers, and supervisors.
* Assists and escorts residents to appointments or activities as needed.
Qualifications
* High School Diploma or equivalent
* Certified Nursing Assistant Certification Preferred But Not Required
* Completion and proof of a PCA/HHA training program that meets the 54 training hour requirement from EOEA
* Previous experience working in long term care/assisted living community preferred
* Previous experience working with dementia residents
