An exciting opportunity has arisen for a Dementia Post Diagnostic Support Worker within the Older Adult Service based at Inverurie Hospital within the Aberdeenshire Health & Social Care Partnership. The post is based in Central Aberdeenshire providing care for individuals who have been referred from GP Practices in the expansive but picturesque catchment area. Car ownership and licence are a prerequisite for the post. We are looking for a motivated and skilled practitioner who has sound assessment skills, ability to communicate well within a team, be able to work independently and have experience of multi-agency working. You should preferably have experience working with older adults, including functional and organic illnesses. Due to the nature of the illness patients can be referred to the service who are under 65 but would still qualify for the support. The successful candidate will manage a caseload under the supervision of the Team & Deputy Team Leader. Informal visits to the team are encouraged.
