Our Health in Justice service line is expanding into non-Custodial healthcare, delivering a person-centred approach through a solid infrastructure to assess individuals’ mental health and neurodiversity needs, and to redirect them appropriately. We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in West Yorkshire.
Hours - Monday to Friday; 8am - 6pm
Some weekend work may be required
Salary - £42,094 to £47,446 depending on experience
Location - West Yorkshire Prison Sites - HMP Leeds, HMP New Hall & HMP Wealstun & Community

Candidates are required to have the legal right to work in the UK due to the nature of the role. Offers of employment will be subject to vetting and security checks.
What you'll be doing
As a Clinical Reconnect Practitioner, your responsibilities will include:
* Working autonomously in prison and community settings to identify and respond to screening and assessment requests for individuals leaving prison within 12 weeks or less.
* Reducing reoffending by addressing health and social needs and ensuring onward referrals for continuity of care. Collaborating with probation and other stakeholders to assess and meet service users' needs effectively.
* Ensuring the needs of service users are met and communicated to relevant parties for continuity of care, onward support, and risk management.

Qualifications and skills required:
* Qualified Registered Mental Health Nurse, Mental Health or Social Worker with current registration
* Experience working face-to-face with patients with complex mental health conditions
* Knowledge of substance misuse services and support options
* Strong clinical assessment and risk assessment skills
* Understanding of multi-disciplinary and multi-agency collaboration
* Non-judgmental, compassionate approach
