Job Purpose
The Care Coordinator is responsible for the effective coordination and delivery of high-quality, person-centred care within domiciliary and extra care services. The role ensures service users receive safe, responsive, and well-planned care that promotes independence, dignity, and wellbeing.
Key ResponsibilitiesService Coordination
* Coordinate and manage day-to-day care service delivery.
* Develop and maintain staff rotas to ensure adequate care coverage.
* Allocate care workers based on skills, experience, and service user needs.
* Ensure continuity of care wherever possible.
Care Planning & Assessments
* Conduct service user assessments.
* Develop, review, and update comprehensive care plans.
* Complete and maintain detailed risk assessments.
* Ensure care plans are person-centred and outcome-focused.
Compliance & Quality Assurance
* Ensure services meet regulatory standards and organisational policies.
* Maintain accurate and up-to-date documentation and records.
* Monitor care delivery and address any quality concerns promptly.
* Support audits, spot checks, and service reviews.
Staff Support & Supervision
* Support care staff with guidance and daily coordination.
* Identify training needs and escalate to management where necessary.
* Assist with induction of new staff members.
Communication & Liaison
* Act as the main point of contact for service users and families.
* Liaise with healthcare professionals, social workers, and external agencies.
* Respond to concerns, complaints, and feedback professionally and promptly.
Medication & Safety
* Oversee safe medication administration practices.
* Ensure health and safety policies are adhered to.
* Coordinate emergency responses and report incidents appropriately.
Person SpecificationEssential
* Minimum 6 months’ experience in domiciliary or extra care services.
* Experience in writing care plans and risk assessments.
* Strong organisational and time management skills.
* Good understanding of adult social care regulations.
* Excellent communication and interpersonal skills.
* Ability to work under pressure and manage priorities.
* Valid DBS (or willingness to obtain one).
Desirable
* NVQ Level 3 in Health & Social Care (or working towards).
* Previous experience in a coordination or supervisory role.
