Adult's Social Worker - Hospital Team - BCP - £36 ph
* Location: Bournemouth (Hybrid – Bournemouth Hospital)
* Rate: Up to £36 per hour
* Contract: Full-time
We are currently seeking an experienced professional to join our team on a hybrid basis, working at Bournemouth Hospital 2–3 days per week. This role requires strong experience within a hospital setting and a solid understanding of Discharge to Assess (D2A) pathways.
Key Responsibilities
* Work collaboratively within a hospital environment to support safe and timely discharges
* Apply Discharge to Assess principles to ensure effective patient flow
* Liaise with multidisciplinary teams, patients, families, and external agencies
* Ensure assessments and decisions are person-centred and outcomes focused
* Maintain accurate and timely records in line with policies and procedures
Requirements
* Proven experience working within a hospital setting
* Clear understanding and practical experience of Discharge to Assess (D2A)
* Ability to work on-site at Bournemouth Hospital at least 2–3 times per week
* Strong communication and organisational skills; Ability to work effectively in a fast-paced environment
