An exciting opportunity has arisen for a PCN Pharmacy Technician to join our ever‑expanding team within Tadcaster and Selby Rural PCN working within the Practices.
Tadcaster and Selby Rural PCN is a well established 3 Practice PCN with approximately 32,000 patients, covering the large geographical area surrounding Tadcaster, Sherburn‑in‑Elmet and South Milford.
The right candidate should be caring, dedicated and compassionate. They will also need an open and flexible attitude towards working with a variety of different patient needs. Good written and verbal communication skills as well as strong time and case management abilities are also needed.
Main duties of the job
The Pharmacy Technician will play an important role within the Practice and complement the more clinical work of the PCN Pharmacists, through the utilisation of their technical skillset. General tasks will include important audits, discharge management, prescription issuing, and other work generated by the PCN Practices and Pharmacists.
The post holder will be supported by Clinical Pharmacists working for the other Practices within the PCN.
The successful candidate should have a passion and drive for providing high quality and personalised care. General Practice experience is desirable but not essential. The post holder will be required to enrol on the General Practice Pharmacy Technician Training Pathway from CPPE if not already completed.
About us
The contract will be held by one of our member Practices on behalf of Tadcaster and Selby Rural PCN. The successful candidate may be working in one or all of the PCN Practices (South Milford Surgery, Tadcaster Medical Centre and Sherburn Group Practice).
Job responsibilities
* Support the PCN Practices with all aspects of medicines optimisation and reconciliation.
* Work within a multidisciplinary team to provide expertise in the medication aspects of the role, alongside GPs, Advanced Clinical Practitioners and other AHPs employed across the PCN.
* Carry out medicines optimisation tasks including effective medicines administration, supporting medication reviews and medicines reconciliation.
* Medication reconciliation from discharge letters.
* Support Clinical Pharmacists in Structured Medication Reviews and outcomes.
* Support the PCN Practices to achieve local QoF, QIPP and other locally commissioned services including the enhanced service.
* Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.
* Support any national vaccination programme in a clinical capacity (COVID vaccinations).
* Telephone medicines support.
* Management of medicines in a care home setting.
* Review safer prescribing and prescribing safety alerts.
* Support care homes with medication requests and management of ERDs.
* Undertake clinical audits and report findings back to the wider Practice and PCN team.
* Demonstrate an understanding of the wider health, social, economic and environmental factors impacting patients.
* Appreciate the nature of Primary Care prescribing, concepts of safe prescribing and strategies to improve prescribing.
Person Specification
Qualifications
* Completion of an undergraduate degree in Pharmacy Technician work and registration with the General Pharmaceutical Council, demonstrating an understanding of and conforming to the relevant standards of practice.
* Experience within General Practice or PCN network setting.
* Successful completion of the Postgraduate Diploma in Clinical Pharmacy.
* Services via the CPPE General Practice Pharmacist Training Pathway.
* Knowledge of SystmOne (clinical system).
