Community Therapy Response Team
Base:
Stratford Location
We are seeking a flexible, dynamic, and enthusiastic Band 3 Therapy Assistant to join our Community Therapy Team, covering the South Warwickshire locality. This service model emphasizes more care in the community, better use of technology, and increased multi-disciplinary collaboration.
Our community services have expanded significantly in recent years, and we are looking for excellent team members to continue our journey of innovation and development.
You will receive strong support and encouragement to develop your skills to meet our service needs and your career aspirations. The service operates from 8:00 am to 6:00 pm.
The successful candidates will work with patients in their homes to deliver integrated interventions, equipment provision, and short-term rehabilitation.
You will liaise closely with Occupational and Physiotherapists.
Since this role involves visiting patients at home, it is essential that applicants are drivers and have their own vehicle.
The post holder will undertake roles delegated by the Community Team Leader or other qualified and regulated professionals. Working within the boundaries set by the regulated professional, the role includes making non-complex assessments of patients according to competence and acting on those assessments. You will collaborate with the team to meet the needs of patients with long-term conditions by providing quality, evidence-based care. The post holder will follow practice policies and procedures and provide supervision to junior staff.
Join a Trust rated Outstanding by the CQC. South Warwickshire NHS Foundation Trust has been rated as outstanding following its latest inspection, and we are recruiting new staff to help us improve even further.
We provide services across Warwickshire and beyond, including hospitals in Warwick, Stratford-Upon-Avon, Royal Leamington Spa, and Shipston-on-Stour, as well as community services across the county. We are a progressive, expanding organization with a strong focus on improving quality, integrating pathways, and increasing productivity.
Our core values are summed up as: "Trusted to provide safe, effective, and compassionate care."
