Norfolk and Suffolk NHS Foundation Trust provides child and adult mental health services, learning disability, wellbeing, older people's and eating disorder services across Norfolk and Suffolk.

We are committed to equality, diversity and inclusion and want to reflect the diversity of our local communities within our teams. We welcome applications from all talented individuals with the relevant qualifications, skills, knowledge and experience

Job overview

The Learning Disability and Autism Liaison Team are recruiting a Band 4 Nursing Associate to work across east and west Suffolk as required, to support the registered practitioner staff in delivering the service.

You will be pivotal in ensuring people with a learning disability have their physical and mental health needs met in primary care services in a timely and appropriate manner. If you are looking for a fulfilling and varied role within an innovative team and you have a genuine desire to develop your career in community learning disability nursing and a commitment to providing high quality care, then we want to hear from you!

The service is provided in GP practices and across various other community settings, including outreach into people's homes and operates from bases in Ipswich and Bury St Edmunds. It operates 9-5pm, Monday to Friday and as part of NSFT's Learning Disability & Autism Specialist Pathway has close links with the community learning disability teams and other teams within primary care services.

Main duties of the job

Are you able to think and work creatively with individuals and their carers? You will be able to work independently, supporting the delivery of person-centred care plans with a focus on prevention of ill health through promotion of healthy lifestyles. Communication and engagement are key for people with learning disability, so these are key skills required along with a positive and creative approach. You will be able to demonstrate that you are able to apply your skills to a wide range of situations, working with a range of people with learning disability in the community.

The team offers:

* Consultation, advice, brief assessment and formulation to GPs on the needs of adults with learning disabilities who may also have associated physical health conditions, behavioural difficulties and/or mental illness.
* Advice and support to individuals and families about health needs.
* Support to develop accurate Learning Disability registers, attendance at annual health checks, and health action planning for people aged 14 and over with learning disabilities.
* Signposting to access the use of health and social care services and other organisations.
* Supporting access through reasonable adjustments for people with a learning disability and autistic spectrum conditions.

Working for our organisation

The way we care for people with Learning Disabilities in Suffolk is changing at NSFT as we have recently benefitted from £1.3 million of investment for our Acute services and the role out of a new Learning Disability and Autism Specialist Service.

The development is fully driven by the national Transforming Care Agenda. The Learning Disability and Autism Specialist Service is not only being invested in financially, with a fresh approach to ideas and service delivery being supported by the Trust, we are confident our new model of care is the very best for the people we care for.

We have a culture of continued learning, working as a close LD specialist pathway with each team having protected days which includes a variety of training, reflection, supervision and service development. The entire pathway is focused on the further development of high-quality care in line with NHS LD standards alongside our colleagues in mainstream services.

Detailed job description and main responsibilities

We place an emphasis on high quality clinical & managerial supervision and continued professional development, as we regard this as a vital part of the delivery of care. You will have the opportunity to be fully involved in further development and direction of the role. We offer a variety of opportunities in a supportive and friendly environment with a dynamic team. You may wish to be part of quality improvement projects or be part of a leadership group planning and implementing the driving change for Transforming Care and Quality Standards for Learning Disability. Here is your opportunity!

You must have a full and valid driving licence and have access to a vehicle due to the community element of the role and the large geographical area that the service covers. The West team is based in Bury St Edmunds and the East team is based in Ipswich.

Benefits included with this role are:

* A comprehensive in house & external training programmes
* Starting annual leave of 27 days per annum increasing to 33 days pa based on length of service (plus paid bank holidays)
