Community Nurse - Specialist Palliative Care Rapid Response Team
Salary: Band 6 AFC
Working Hours: Approx 150 hours over four weeks. Worked in 12 hours shifts inclusive of weekends and public holidays.
We are looking for an experienced, enthusiastic, andhighly motivated individual to join our rapid response service. This post is suited to a senior nurse with experiencecaring for patients with life limiting illnesses who has skills in holistic palliativecare assessment, symptom management and advanced communication.
You will be a key member of the rapid response team, and the widermultidisciplinary team working across Royal Trinity Hospices communitycatchment area supporting patients with urgent and complex specialistpalliative care needs wherever they call home.
The RapidResponse Service is a 7-day service operating 08:00-20:00
During those hours the rapid response teamsenior nurse will respond urgently to referrals identified as appropriate forthe service across Trinitys catchment area. They will carry out rapidspecialist palliative care assessments and develop an immediate care andsupport plan for patients urgent and unaddressed specialist palliative careneeds.
Main duties of the job
To support the functions of the ClinicalHub including:
* First contactteam
* Specialist Community Nursing
* To provide nursing care within any existing care plans, and inaccordance with the Clinical Nurse Specialist, and medical care prescribed bythe GP
To provide specialist nursing care and support to the families andcarers of individual patients in the place they call home across the Trinitycatchment area, being sensitive to their changing physical, psychosocial andspiritual needs.
To communicate and liaise with the patients usual care team includingDistrict Nurses, other palliative care support such as the 111 Roaming Serviceas well as the Clinical Hub Team Lead, reporting on the condition and needs ofthe patients and providing and maintaining clinical records, as necessary.

Job responsibilities
Overall Purpose of the Role
The Rapid Response Service is a 24/7 service delivered across two shifts: 08:00-20:00 and 20:00-08:00.
During those hours the Rapid Response Nurse and a Palliative Care Assistant (PCA) will respond tothe needs of patients identified as appropriate for the service across Trinitys catchment area byproviding specialist palliative nursing and end of life care.
Key Tasks:
* To support the functions of the Clinical Hub including:Triage and first contact
* The functions of the 24-hour specialist palliative care advice line
* Virtual Hospice Ward
* Specialist Community Nursing
* To provide nursing care within any existing care plans, and in accordance with the ClinicalNurse Specialist, and medical care prescribed by the GP.
* To provide specialist nursing care and support to the families and carers of individualpatients in the place they call home across the Trinity catchment area, being sensitive totheir changing physical, psychosocial and spiritual needs.
* To communicate and liaise with the patients usual care team including District Nurses, otherpalliative care support such as the 111 Roaming Service as well as the Clinical Hub TeamLead, reporting on the condition and needs of the patients and providing and maintainingclinical records, as necessary

Qualifications
* Professional qualification and associated membership of nursing profession.
* Registered with the NMC
