About the Role
We are seeking an experienced Clinical Service Manager to lead and oversee high-quality, person-centred homecare services for adults and young people with complex physical health needs.
This role provides clinical governance, service leadership, and operational management across a caseload of individuals supported in their own homes.
You will ensure that care is safe, effective, and compliant with CQC regulations, supporting packages that may include ventilation, PEG feeding, tracheostomy care, spinal injuries, neurological conditions, degenerative disorders, and other complex physical conditions.

Responsibilities
Clinical Governance & Quality Assurance
* Provide clinical oversight for all complex physical care packages delivered in clients homes.
* Ensure compliance with CQC standards for domiciliary care, safeguarding, MCA/DoLS, and relevant legislation.
* Lead on clinical risk assessments, care plans, and person-centred health support plans.
* Monitor medication administration, clinical procedures, and competency sign-off for care staff.
* Investigate incidents, manage safeguarding concerns, and implement corrective action plans.
* Conduct regular clinical audits and service quality reviews.
Service Leadership & Operational Management
* Oversee the safe delivery of homecare services across multiple clients and locations.
* Support care coordinators in rostering, capacity management, and matching staff to clients safely and appropriately.
* Participate in assessments for new referrals, liaising with commissioners, hospitals, and multidisciplinary teams.
* Ensure timely mobilisation of new care packages, including training and competency assessments.
* Collaborate with community nursing teams, hospitals, physiotherapists, occupational therapists, GPs, and commissioners.
* Attend MDT reviews, discharge planning meetings, and case conferences.
* Ensure effective communication with families and advocates, promoting shared decision-making.
Workforce Management & Development
* Line-manage senior carers, nurses (if applicable), and care staff providing complex physical care.
* Ensure staff are trained and competent in specialist tasks such as:
o Tracheostomy care
o Ventilation support
o PEG/enteral feeding
o Catheter and continence care
o Moving and handling
o Pressure care
* Support ongoing professional development through clinical supervision, competency reviews, and reflective practice.
Performance & Compliance
* Maintain detailed and accurate clinical documentation.
* Monitor KPIs such as care continuity, incident trends, client outcomes, and training compliance.
* Support quality improvement projects and contribute to business development where appropriate.
Requirements
* Registered clinical professional (e.g., NMC, HCPC, GPhC) OR significant experience in managing complex physical homecare services.
* Previous experience as a Clinical Lead, Complex Care Manager, or Service Manager in homecare.
* Experience supporting both adults and children with complex health needs
* Demonstrated experience in complex physical care, such as spinal injuries, neurological conditions, long-term ventilation, degenerative diseases, or high-acuity care.
* Strong understanding of CQC regulations for domiciliary care and clinical governance.
* Experience in leading and managing multidisciplinary or care teams in the community.
* Ability to conduct and sign off clinical competencies for staff.
* Excellent organisational, communication, and risk-management skills.
* Ability to work flexibly and travel across the North West service area.
Desirable Requirements
* Knowledge of ventilation training, tracheostomy management, or enteral feeding procedures.-
* Leadership or management qualifications (ILM, registered manager qualifications, etc.).
* Experience managing hospital-to-home transitions.
