Activity Co-ordinator - Campion Ward and Nicol Unit

Closing date: 30 May 2026

Must be able to work at both Campion Ward (Leamington Rehab) and Nicol Unit. Both wards are GP run and care for patients requiring sub-acute admission prevention, rehabilitation or palliative care.

The post holder will work as an integral member of the multidisciplinary team within a community hospital rehabilitation setting, supporting patients to regain and maintain independence through activity promoting reablement, rehabilitation and recovery. Experience with dementia patients is desirable.

Main duties of the job

* Provide a range of creative, social and meaningful activities to stimulate and engage a diversity of individuals and groups of patients.
* Maintain a welcoming, safe and positive physical and emotional care environment.
* Work collaboratively with other members of the multidisciplinary team (MDT).
* Carry out specific tasks involving direct patient care without the direct supervision of registered practitioners.
* Exercise autonomy within parameters agreed with the line manager to achieve the above.
* Understand and meet the needs of individual patients using a person-centred approach and encourage and motivate others to do the same.
* Promote effective communication with all patients/clients, colleagues, other health care staff and visitors to the Trust, always projecting a professional image.

Delivery of Direct Patient Care (after completion of in‑service training)

* Dressing and undressing patients as part of therapeutic intervention.
* Assisting patients with toileting during activity sessions (including urinals, commodes, bed pans).
* Supporting patients to meet dietary and fluid requirements and reporting concerns to registered nurse.
* Participating in moving and handling of patients/clients.
* Promoting infection prevention & control practices.
* Preparing for and participating in food service, encouraging socialisation at mealtimes.
* Providing one‑to‑one care when required under trust guidelines.


Therapeutic Activities

* Plan and implement individual and group‑based therapeutic activities that support rehabilitation goals and promote independence.
* Organise and deliver structured physical, functional and social activities to improve confidence and performance in everyday tasks.
* Collaborate with occupational therapists and physiotherapists to integrate therapy goals into activity sessions.
* Engage patients in a dementia‑friendly ward environment that promotes engagement, orientation and wellbeing.
* Facilitate personalised activity sessions to reduce risks such as deconditioning, falls and social isolation.
* Assist in coordination of social dining interventions and support dignity, independence and therapeutic outcomes during mealtimes.
* Maintain effective communication with MDT and accurately report patient progress, observations and changes in function.
* Coordinate with dementia specialists to support patients living with dementia and their carers.
* Utilise person‑centred care tools (e.g. This is Me, Forget‑Me‑Not Scheme). Monitor implementation of the Trust's Dementia Care Bundle.
* Communicate compassionately with patients experiencing confusion, cognitive impairments, delirium or mental health challenges.
* Assist ward staff in maintaining a safe, orderly, clean and tidy working environment.
* Keep activity resources clean, tidy and stored; ensure all items accounted for and prevent theft of Trust property.


Administrative Duties

* Answer the telephone in the required manner for all Trust employees.
* Complete patient charts including SKIN bundle, Intentional Rounding, food and fluid charts, bowel charts, pain assessments.
* Document interactions with patients, visitors and carers appropriately.
* Individualise care plans for patients with specific planned activities.
* Ensure availability of “This is Me” and Forget‑Me‑No Scheme leaflets to visitors and carers.


Team Working

* Develop positive working relationships with medical, nursing and therapy teams.
* Work with students, volunteers, apprentices and work‑experience individuals to provide a welcoming learning experience.
* Collaborate with relatives and carers to prevent and resolve complaints at a local level. Report concerns to Nurse in charge and offer visitors PALS information if unable to resolve.


Personal Development

* Identify and develop skills needed for the role; seek improvement in knowledge relevant to clinical areas and patient groups.
* Maintain all mandatory and statutory training as required by the Organisation within relevant time intervals.


Person Specification


Qualifications

* Basic level education (numeracy and literacy).
* Commitment to further professional experience/knowledge.
* Level Three NVQ completed or equivalent or working towards.
* Dementia awareness training.
* Computer literacy / basic IT skills.
* Awareness of Safeguarding Vulnerable Adults, young people and children responsibilities.
* First Aid at Work Certificate.
* Enhanced Dementia Training.
* Qualification in Reminiscence Therapy.


Experience

* Previous experience in a care or therapy setting.
* Experience of working with people with special requirements such as impaired sight or hearing, other disabilities.
* Evidence of ability to maintain effective working relationships with colleagues, patients and visitors.
* Activity co‑ordination / group reminiscence experience.
* Experience with multi‑professional team.
* Experience of working with persons with dementia.
* Experience of organising group activities, outings, parties etc.


Skills

* Able to undertake a range of physical care interventions as directed by qualified staff.
* Ability to accurately pass on information clearly and precisely.
* Able to promote professional image.
* Awareness of confidentiality requirements.
* Ability to work in a team and independently.
* Able to carry out delegated duties without direct supervision, prioritise and organise own workload.
* Skills in arts/crafts – drama, creative activities, art/drawing, music, local history.

* Full clean driving licence and access to a car.
* Flexible in working shift patterns and adapting with the needs of the service which may include evenings or weekends, bank holidays and within the acute site in all departments as needed.
