Your Role
This role includes completing Care Act assessments, support planning and risk assessments under the Care Act 2014. You will also undertake mental capacity assessments, produce social circumstance reports, and work closely with the community mental health team and other health colleagues. The role requires a team‑oriented approach with the ability to work autonomously, experience in mental health social work, knowledge of the Mental Health Act, safeguarding enquiries, and a strengths‑based practice style.

We are looking for a qualified and experienced social worker with a passion for mental health social work, who is motivated to deliver care, maintain service user independence, and use community resources in an asset‑based approach.
