Our service users all live in their own homes and have Learning Disabilities; some also have health diagnoses. Your role will involve supporting their health needs and promoting health and independence.
Each service user is supported by a dedicated team. This is an excellent opportunity to work within the challenging yet rewarding field of community-based learning disability services alongside a multidisciplinary team.
This Team Leader position is a full-time, Monday to Friday role. We operate a 24/7 on-call system to support healthcare assistants. You will be required to participate in this rota, which includes an uplifted salary and overtime payments for out-of-hours calls. A valid driver’s license and at least 2 years' experience as a Band 5 Nurse are essential.
You will demonstrate a commitment to working proactively and in partnership with people with learning disabilities, using your professional knowledge to support individuals within the community.
You will work directly with people with learning disabilities in their homes, collaborating with their families, healthcare assistants, and other professionals. As a Team Leader, you will oversee at least two services.
All practice should be based on the principles of Valuing People—rights, independence, choice, and inclusion. You will work with individuals and their carers to develop, implement, and review person-centred care plans and risk assessments.
As a Team Leader, you will support healthcare assistants, providing clinical expertise, guidance, training, supervision, and managing sickness. You will also assist with shift cover using the e-roster system and support staff with various issues as they arise.
We provide a 24/7 on-call service, shared equally among team leaders and management, planned one month in advance.
