Overview
Indigo – Care Navigator (0.4 FTE) is a part-time role on a fixed-term basis, with hybrid working (ideally one day in our Manchester office). Salary: £27,927 (pro-rata) + 10% pension. Accountable to: Indigo Care Navigator Team Lead.

We are an LGBT Foundation charity with a focus on trans and non-binary communities. Indigo Gender Service provides a primary care led partnership to enable trans, non-binary and gender variant people to reach their full potential. The role focuses on non-clinical support, administration, and advocacy to ensure an empathetic, trans-affirmative service.
Ethos and Equality
We value lived experience of being trans and/or non-binary and having used NHS gender transition services. We are taking positive action to encourage applications from people of colour (PoC) and other racially minoritised communities, trans*, non-binary, and/or older people (aged 50+).
* Trans is an umbrella & inclusive term describing gender identity variations.
Role Accountabilities
* To deliver an effective navigation service that provides practical, emotional and social support for trans and non-binary people, including in-person and remote contact.
* To travel within the Greater Manchester area to meet patients at convenient locations.
* To act as a point of contact for service users, ensuring regular reviews and progress toward individual goals.
* To communicate with empathy, understanding, diplomacy, honesty and integrity.
* To support self-management by signposting to appropriate information and resources.
* To understand safeguarding principles and apply them in practice.
Administration for Indigo Gender Service
* Provide administrative support to the clinical team and maintain up-to-date information.
* Manage appointment bookings, changes and cancellations.
* Ensure connectivity between patient-facing services and back-office systems.
* Coordinate and follow up referrals to treatment and support services, including voluntary and community organisations and other agencies.
* Support patients by answering the service’s helpline.
Development of the Trans Health Service
* Collaborate with NHS and voluntary sector groups to improve access for marginalised groups, including those with language barriers or disabilities.
* Maintain knowledge of Indigo Gender Service and related support in Greater Manchester.
* Build strong working relationships with external and internal partners and participate in effective teamwork.
* Provide practical solutions to improve quality of life and engage in co-production with trans and non-binary people for service development.
* Commit to continuing professional development and share good practice.
* Provide advocacy and liaison between Indigo Gender Service and patients’ GPs.
