The Stoma Care Team is a small team consisting of
1 Band 7
3 Band 6's
1 Band 2.
We cover both inpatients and Community over the East Lancashire area including adult and paediatric patients.
The role varies from teaching/supporting and advising patients with stomas.
Main duties of the job
* To manage a caseload of new and existing patients providing specialist knowledge, advice, care and support in the hospital and community for patients with Colorectal/Inflammatory Bowel disease and/or Urological disorders leading to Stoma formation.
* The role requires cross boundary interface working across East Lancashire Hospitals NHS Trust. This is in collaboration with East Lancashire and Blackburn with Darwen commissioners in order to provide seamless care for the patient, families and carers in the hospital setting and community.
* To provide Stoma Care Nursing advice to Health Care Professionals/Allied Health Care Professionals/Doctors and Nurses both in the hospital and community setting.
* To teach and educate all health care professionals, patients, carers and families on all aspects needed for stoma management.
* Adhere to ASCN guidance to implement best practise guidance and improve the delivery of care.
* To liaise with the East Lancs Appliance Prescription Service to assist with patient queries and support the service in offering guidance on how to order products and detect continence and stoma needs.

Detailed job description and main responsibilities
* The post holder is expected to have direct clinical involvement in and have extensive knowledge of caring for people with colorectal cancer/inflammatory bowel disease and urological disorders.
* Provide essential advisory and psychological care to patients with stoma formation, inflammatory bowel and colorectal disease, and urological disorders with special responsibility for patient advocacy.
* Teach patients, carers and health care professionals how to change stoma appliances.
* Develop and implement clinical standards to underpin practice and evaluate these regularly.(ASCN )
* Provide pre-operative stoma siting with regard to the patient’s physical life style, cultural and religious beliefs.
* Promote an environment which facilitates the exchange of information between patient/carer/nurse.
* Provide stoma specialist advice to patients, clinicians and health care professionals.
* Plan, implement and review programmes of care for patients who have received treatment for colorectal cancer, inflammatory bowel disease and urology disorders resulting in stoma formation, making relevant recommendations regarding their ongoing care.
* Work collaboratively with other relevant practitioners, i.e. GPs/ Colorectal Specialist Nurses/colorectal MDT member/District Nurses/Specialist Palliative Care Nurses/paediatric CNS’s/Appliance Prescription Service.
* Provide pre-operative and post-operative care for all stoma patients both hospital and community.
* Promote and follow infection control policy. Adhere to hand washing procedure.
* Recognise and respond to emergency situations.
* Participate with recruitment and selection of staff
* Assist in appraisals and developmental plans
* Maintain fitness to practice as per NMC guidance
* Ensure NMC membership is annually renewed
* To work within the guidelines of the NMC policies and procedures
Education Responsibilities
* Plan, develop and implement appropriate education and training programmes for patients and other health care professionals.
* Be responsible for professional and personal development and continuing education to meet PDR/NMC
* Develop skill and expertise of hospital and community based nurses.
* Teach and educate ward based learning and Stoma Link Nurses.
* Provide Health Promotion and education for patients.
* Through PDR objectives identify own learning needs and how these can be met.
* Attend conferences (ASCN ) and sponsored events to update knowledge in the field of stoma care nursing
Research
* Ensure professional practice is evidence based against best practise guidance. (Medtech Europe ) (ASCN:2018 )
* Maintain awareness and review literature relevant to specialist practice.
* Participate in and/or initiate research studies with appropriate ethical approval.
* Seek out new ways of working and challenge practise
Management Responsibilities
* Complete and maintain accurate patient records to meet the requirements of Nursing & Midwifery Council Standards for Record Keeping.
* Provide efficient systems of referral and review systems of documentation.
* Organise workload to take account of priorities of care in meeting the needs of a defined caseload
* Attend relevant meetings and discussions as required.
* Develop and implement operational policies/protocols as appropriate
* Work within all NMC guidelines for practice including the Scope of Professional Practice.
* Be aware of Trust policies and procedure and Trust business plan/objectives.
* Contribute to the agenda of the Directorate and the Trust
* Maintain active status on NMC register.
* Act always in accordance with NMC Code of Conduct and guiding documents.

* Extensive post-registration experience, ideally minimum 3 years
* Evidence of excellent interpersonal and communication skills
