Overview
We have an exciting opportunity for a dynamic, forward-thinking Registered Nurse to join the Pre-Admission Discharge Lounge Team at NHS Golden Jubilee Hospital. The role includes liaising with wards and external agencies to ensure social care packages are in place prior to discharge. The successful candidate will carry out simple discharge referrals to external agencies such as Community Nurses and Social Services and contribute to the wider discharge service.
Responsibilities
* Work autonomously over the shifts and interact with patients and relatives in a calm and reassuring manner.
* Contribute to the wider discharge service and coordinate with wards and external agencies to ensure discharge planning needs are met.
* Demonstrate a caring and compassionate demeanor, good communication skills, and a proactive "Can Do" attitude.
* Prioritise conflicting demands to deliver patient and service needs.
Qualifications
* Registered Adult Nurse with a First Level nursing degree (or equivalent). As per NMC registered nurses.
