Job Summary
The East Lancashire Alliance is recruiting a care coordinator to join the team within the Hyndburn Central PCN. The role will involve working with the various practice sites across Hyndburn central and may interlink with other teams such as Pharmacists. It is an initial fixed-term contract of 2 years, with potential extension or conversion to permanent. The post holder must be flexible in working pattern to support all practices in the PCN according to patient needs.
Key Responsibilities
* Provide wide-ranging and efficient administrative support to the Hyndburn Central Primary Care Network (PCN), including arranging meetings, managing correspondence, document management, preparing short reports, creating spreadsheets, and responding to and forwarding e‑mails.
* Take minutes of PCN meetings and support the PCN Administration Team in coordinating care for patients registered with practices within the Hyndburn Central PCN and care homes.
* Coordinate and liaise with healthcare professionals, non‑clinical staff and volunteers to support patient care within the network.
* Support the delivery of quality patient services by working closely with GP’s and other primary care professionals within the PCN.
* Play an integral role in facilitating multidisciplinary teams (MDTs) to improve continuity of care, acting as a point of contact for residents, families and professionals.
* Work with primary care teams to develop and implement data collection systems that provide accurate and timely data for monitoring and evaluation of services.
* Act as the first point of contact for patients and clinicians in coordinating patient care, including organising staffing rotas, completing patient referrals and supporting day‑to‑day provision of the Enhanced Access service.
* Deal with incoming queries from patients, carers and other healthcare providers, and encourage feedback and engagement in the care coordination process.
* Collaborate with our Care Homes to support flu and COVID vaccinations and other health services.
* Work proactively with other PCN Additional Roles staff and the Federation team to identify patient service needs and inequalities, proposing projects or initiatives to support patient care.
* Support the coordination and delivery of MDTs and assist the PCN Transformation and Development Manager with administrative duties, planning and organising meetings, taking minutes and ensuring documentation is shared and uploaded appropriately.
* Assist the PCN Clinical Director in collecting information from member practices, identifying anomalies, summarising findings and undertaking enquiries for the PCN and the wider group as necessary.
* Monitor tasks to ensure completion and provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers and other primary care professionals.
* Signpost and organise appointments, follow‑ups or other actions to help the PCN provide high‑quality, compassionate care to the patient population.

Qualifications

Experience
