We are looking for a highly motivated and empathetic individual with a passion for making a positive impact on people's lives.

As a Proactive Case Worker, you will be responsible for managing a caseload of individuals, providing comprehensive support and financial guidance to help them overcome challenges and achieve their goals. You will adopt a proactive approach, identifying needs, coordinating services, and building strong relationships with clients and external agencies.

Key Responsibilities:

1. Manage a diverse caseload, conducting assessments and developing tailored action plans.
2. Provide ongoing support, advocacy, and signposting to appropriate services.
3. Work collaboratively with clients to set achievable goals and monitor progress.
4. Maintain accurate and up-to-date case notes and records.
5. Collaborate effectively with internal teams and external partners to ensure holistic support.
6. Participate in team meetings and contribute to service development.
Adhere to all relevant policies, procedures, and safeguarding guidelines.

About You:

7. Proven experience in a case management or similar role.
8. Excellent interpersonal and communication skills, with the ability to build
rapport and trust.
9. Strong organisational skills and the ability to manage a demanding workload
efficiently.
10. A proactive and solution-focused approach to problem-solving.
11. Empathy, sensitivity, and a non-judgmental attitude.
12. Ability to work independently and as part of a team.
13. Proficiency in IT systems and record-keeping.
