Job overview

This is a senior role within the Integrated Discharge Team based primarily at Winchester but with the requirement to travel to Andover and Basingstoke in order to provide adequate nursing cover.

The team works with the multi-disciplinary teams within HHFT as well as with partners from the community to identify appropriate ongoing care for some of our most vulnerable adult patients and to manage the processes which expedite safe and efficient person centred discharge. The person will have knowledge and understanding of discharge pathways with a real interest and ability to facilitate conversations with system partners, wards, clinicians about appropriate onward care settings. They must be a skilled communicator with the ability to explain complex arrangements to patients and families as well as perform a teaching role on the wards.

Main duties of the job

The role includes liaison, teaching and negotiating with other professionals inside and outside the Trust, as well as with patients and their families. We need someone with a calm and confident approach, used to having challenging conversations introducing difficult concepts, and also with a good knowledge and ability with computer systems. The ideal candidate is usually someone who has tenacity and resilience and enjoys solving puzzles, someone who can work independently but also as an integral part of the team.

Qualifications & Training

Essential criteria

1. •Registered Nurse (Adult)
2. •Diploma level 1
3. •Relevant specialist course or equivalent experience
4. •Recognised Teaching/mentorship qualification

Desirable criteria

5. •Degree
6. •Experience of leading & developing a team

Experience & Knowledge

Essential criteria

7. •Post registration experience at band 5 or 6 in the acute or community setting
8. •Experience of complex discharge planning & processes
9. •Experience managing own workload & resources effectively.
10. •Ability to function as an autonomous professional
11. •Knowledge of related legislation & guidance, including The Mental Capacity Act & National Framework for NHS Continuing Healthcare & Funded Nursing Care

Desirable criteria

12. •Experience of project management
13. •Sound knowledge of caring for patients with complex needs including End of Life.
14. •Experience of working with the NHS Continuing Healthcare process

Skills & Ability

Essential criteria

15. •In-depth knowledge of key discharge services
16. •Ability to communicate with all stakeholders at appropriate level, including negotiation skills
17. •Ability to conduct data analysis, write reports and deal with issues of implementing policy
18. •Be competent in working under pressure or in challenging situations
19. •Keyboard skills with knowledge of standard databases and Excel
20. •Problem solving
