We have established a new community rehabilitation team to support individuals with serious mental illness and complex needs. The community mental health rehabilitation team will provide a 7-day service for residents of North Lincolnshire. This service aims to deliver intensive community rehabilitation closer to home, within their local community.
The service comprises a multidisciplinary, multi-agency workforce, collaborating between Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) and North Lincolnshire Council. This partnership allows sharing of skills and expertise to support individuals and enhance their quality of life.
We are seeking an experienced Registered Mental Health Nurse (RMN) to join us as a Band 6 Community Mental Health Nurse. The ideal candidate will be enthusiastic, highly organized, and capable of providing high-quality nursing care, assessment, and treatment.
The successful applicant will deliver comprehensive Community Mental Health Nursing services to a diverse client base experiencing mental health issues, engaged in the community rehabilitation pathway. Collaboration with other agencies is essential, requiring the ability to work independently and as part of a team.
Applicants must demonstrate post-registration experience as an RMN, including inpatient and community settings.
The role includes the following duties (not exhaustive):
1. Providing a patient-centered, accessible, and responsive clinical service over a 7-day week.
2. Delivering skilled nursing care, including assessments, care planning, biopsychosocial interventions, risk assessment, and promoting recovery and inclusion.
3. Administering medicines in accordance with Trust policies and legal requirements.
4. Performing clinical procedures, training, and supervising junior staff.
5. Maintaining professionalism and adhering to NMC guidelines.
6. Contributing to infection prevention and control practices.
7. Respecting patient, carer, and family views, working collaboratively to meet their needs.
