Responsibilities
As a Care Coordinator your key responsibilities will include:
* Coordinating/Managing the care professionals and customer schedules.
* Providing Support
* Liaising with customers, their family, doctors, nurses, and social workers.
* Covering care assistant holiday and sickness
* Participating with an on call rota.
